FLU VACCINATIONS

BOOK NOW FOR YOUR 17/18 FLU JAB

 

We are now booking appointments for our eligible patients for their influenza vaccinations.

 

We have a clinic on Saturday morning - 23rd September and various dates throughout the week.  In addition we will provide a drop in service from 25 September onwards

 

Remember our clinicians have access to your notes and past history

 

The Department of Health criteria for free vaccinations is:

 

Over 65s

Patients in the clinical at risk category:

  • Chronic Respiratory Disease
  • Chronic Heart Disease
  • Chronic Kidney Disease
  • Chronic Liver Disease
  • Chronic Neurological Disease
  • Diabetes
  • Immunosupressed
  • Asplenia or Dysfuction of the Spleen
  • Pregnant Women
  • Obesity (class 111)

As Dept of Health criteria is always changing please contact the surgery to check for eligibility.

 

Dispensary

With effect from Monday 7th November 2016 we are changing the way in which we dispense to our patients who live in Shardlow, Aston on Trent and Weston On Trent from our Boulton Lane surgery.

 

Our dispensary at the main surgery on Boulton Lane will be open between 9.00am - 1.00pm and 2.00pm - 6.00pm.  Patients who collect their dispensed medication from Boulton Lane will do so directly from the dispensary (situated just past waiting room 1).

 

We hope by doing this that we will improve our patient service and accessibility to your medication.

 

Aston on Trent surgery will still operate on the same basis.

 

The process for patients from the Alvaston area who collect their prescriptions remains the same.  We also still have our delivery service.

PATIENT TRANSPORT [ AMBULANCE] SERVICES

Derbyshire Non-Emergency Patient Transport Service (NEPTS).

 

The current provider is NSL but the service will be provided by East Midlands Ambulance Service NHS Trust (EMAS) from 1 August 2016. The NEPTS is for patients who need non-urgent medical or clinical support to get to and from their healthcare appointments.

 

From 1 August onwards, all bookings must be made by contacting EMAS on 0300 300 3434 or completing an ‘on-line’ booking system

 

The onus will remain on patients to book their own transport

 

Entitlement to transport will depend on whether the patient meets the eligibility criteria set out by the local clinical commissioning group. All patients will have to answer a series of questions when booking their transport

 

The eligibility criteria states that NEPTS is available for patients who:

 

  • require assistance from skilled ambulance staff e.g. require access to oxygen whilst travelling
  • have a medical condition that would prevent them from travelling to hospital by any other means e.g. their car or public transport
  • have a medical condition that might put them at risk from harm if they were to travel independently
  • have treatment with side effects that requires support from skilled ambulance staff.

Note: Wheelchair patients will be required to travel in an EMAS wheelchair, but will be able to use their own wheelchair if this is necessary for medical reasons.
