Summary Care Records.

Many of you will have received letters regarding this.

The Summary Care Record is a record of your Name, Address, Allergies, Sensitivities and Medications which are listed in your medical records.  The Summary Care Record can be viewed by Hospital clinicians and Out of Hours Services (ONLY WITH YOUR PERMISSION) should you need to be treated.

If you DO NOT wish your details to be accessed by the Hospital and Out of Hours Service please complete the form you have been sent and return it to the address provided or to the surgery.

If you have no objection to your details being accessed then you do not need to do anything at all, you can destroy the letter.

YOU ONLY COMPLETE  AND RETURN THE FORM IF YOU DO NOT WISH YOUR DETAILS TO BE ACCESSED BY THE HOSPITAL AND OUT OF HOURS SERVICE
