New Patient Registration

We are currently registering new patients

What do I need to do?

Firstly, please use the catchment area tool below to see if you reside within our practice area.

You can check our practice boundary by clicking on the ‘Catchment Area Tool’ below. This will take you to a new window where you can check if your postcode is within our area. You may also want to refer to ‘The Streets We Cover‘.

Please call us if you are unsure

Once you have confirmed that your home address is within our practice area, please complete our registration pack. Please ensure you complete all sections on the two forms to ensure there is no delay or error in your registration. You can collect the registration forms from Reception.

Once you have completed your forms, please hand them to our reception team along with photo ID. We need photo ID in order to offer you access to our on-line appointment and repeat medication booking system.  However if you do not have ID then we can still register you. Registration usually takes up to 7 days however we endeavour to complete the registration as soon as possible. It would also be useful, at this point, if you made us aware of any medications you take and any medications you will need. This can sometimes take a little longer than usual if we don’t have access to your medical records straight away.

New patients are eligible for a new patient health check. Please let us know if you would like one.

All of our patients are allocated a named GP who will be accountable for their care whilst registered with the Practice. If you would like to know who your named accountable GP is please email us. This does not restrict you in any way from seeing other clinicians at the practice.

Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
