Blood Test Clinics

The Pathology department at the Royal Derby Hospital provide Phlebotomy (blood test) Clinic sessions at four hospitals.

Requesting a letter

Patients who are currently applying for services and requesting To Whom It May Concern letter for extra suport in their application process.

For example: 

  • Travel letters for carrying medication
  • HGV/PSV/LSG/Taxi medicals
  • Firearms applications
  • Adoption and Fostering 
  • Holiday cancellations claim form
  • Medical Reports

  • If you booked via the e-referral service you can track your referral here
  • If your referral is with Royal Derby Hospital you should contact them directly by phone on 01332 340131 or via their website

Care Coordinator

Care coordinators are employed by Derbyshire Community Health Services and work within GP practices in South Derbyshire as part of the Community Support Team (CST), to support and enhance integrated care delivery in the community.
