When you reach 13 years of age, it is felt that most people are able to make decisions about their own health.  This doesn’t mean you have to, and we would very much encourage you to talk to your parents, guardian, or responsible adult in your life with any concern and to attend appointments with them.

However, to help protect your personal information, if you have already been signed up for an online account or text messaging (this may have been done by an adult for you from birth) you may want us to change this to your contact details or stop all access.  If you would like to do this please contact the surgery and talk to one of our receptionists who will be happy to help you.  This is to make sure that no-one else (like a parent or guardian) will know when you book an appointment unless you want them to.  You can however leave everything the same as it is if you want.   Should you wish to change access please contact the surgery.

When you are 16 online access and SMS reminders will automatically be stopped.  We will remind you about a month before your birthday.

If you would like to still have access to text messaging and online services (so you can cancel appointments, update your personal details, look at your medical record and much more), you can pop into reception with something that confirms who you are (identification / ID) like a passport, letter from the bank / school or European Health Insurance or National Insurance card and tell us what telephone number and email address you would like us to send information to.

Standard Web Services Access

With Standard Web Services Access you will be able to do the following for yourself and for your family members:

  • View available appointments and select the date and time that is most convenient for you
  • Order your repeat medication and send us short medication-related queries

With Web Access, you will no longer need to wait until we’re open to ring for an appointment or wait on the ‘phone, while we attend to other patients

For a Standard Web Services Access registration letter,  please just ask at the reception desk when you next visit Sycamore

You may request standard services access for yourself and for any family member, including older adult relatives

Equally, you can book your first appointment online by pre-registering here and request a registration letter when you attend

Full Medical Records Access

We actively encourage all patients over the age of 18 years to gain password access to their full medical records

There are two steps involved:

Step 1: Please read this information sheet

Step 2: Print-off and sign this form. Bring it with your photo ID and one proof-of-address

We then authorise full records access on our systems and issue you with a registration letter, if you need one
